Since this is voluntary member-paid coverage, premiums will be billed to you directly by the Voluntary Benefits Coordinator, USI Affinity.

Accidental Death and Dismemberment (AD&D)

Accidents can happen to anyone. In fact, accidents are the leading cause of death for people under age 35 and the fifth leading cause of death for people of all ages.* In addition, an estimated 10.7 million workers suffered disabling injuries on and off the job in 2001.** Fortunately, you can help protect yourself and your family form the financial impact of an accident or serious injury with AD&D insurance.

When you enroll for Term Life insurance, you are automatically enrolled in AD&D insurance. The benefit amount for AD&D insurance is equal to the benefit amount of your Term Life coverage. Since this is voluntary, member-paid coverage, premiums will be billed to you directly by AFT’s Voluntary Benefits Coordinator, USI Affinity.

After carefully considering your lifestyle, you can decide how much life insurance coverage is right for you. You can choose coverage in the amount of up to six times your base annual earnings (up to $1,000,000). To apply for coverage, simply print the application in its entirety and return it to USI Affinity at P.O. Box 505 Matawan, NJ 07747.
