Since this is voluntary member-paid coverage, premiums will be billed to you by the AFT’s insurance administrator, Future Planning Associates, Inc.
What are my options? After carefully considering your lifestyle, you can decide just how much life insurance coverage is right for you. You can choose coverage in the amount of up to six times your base annual earnings. What are the maximum amounts I can enroll for? Accidental Death and Dismemberment (AD&D) Accidents can happen to anyone. In fact, accidents are the leading cause of death for people under age 35 and the fifth leading cause of death for people of all ages.* In addition, an estimated 10.7 million workers suffered disabling injuries on and off the job in 2001**. Fortunately, you can help protect yourself and your family from the financial impact of an accidental death or serious injury with AD&D insurance. When you enroll for Optional Life insurance, you are automatically enrolled in AD&D insurance. The benefit amount for AD&D insurance is equal to the benefit amount for Optional Life coverage. Since this is voluntary member-paid coverage, premiums will be billed to you by the AFT’s insurance administrator, Future Planning Associates, Inc. *National Safety Council®, Injury Facts®, page 10, 2002 Edition.

AD&D coverage is provided under a group insurance policy (Policy Form GPNP99) issued to the American Federation of Teachers by MetLife. AD&D terminates when a member’s employment ceases, when a member’s contributions cease or upon termination of the group contract. |
