AFT provides non-contributory Optional Life and Accidental Death & Dismemberment coverage in the amount of $12,000 during a member's first twelve months of membership. Once the AFT is aware of a new member, an activation package will be sent to the member for this non-contributory coverage. This $12,000 of Optional Life and Accidental Death & Dismemberment coverage will become effective on the first day of the month after the activation form is received by the AFT Insurance Administrator. The member will receive under separate cover information about other options for Optional Life and Accidental Death & Dismemberment coverage that are available as an AFT member.
Members can enroll in additional amounts of coverage without a Statement of Health for a period of up to 12 months from the date of hire as long as you have not been hospitalized or treated for any condition within the prior 90 days and are considered to be an active employee.
